NO PARTICIPANT WILL BE ACCEPTED WITHOUT COMPLETELY FILLING OUT THE INSURANCE PORTION OF THE REGISTRATION / APPLICATION MEDICAL AUTHORIZATION.
To permit treatment of injuries, the following authorization must be signed and dated by the participant's parent(s).
In an emergency, I hereby give permission for my child to be examine by a program coach.
I also give permission to an athletic trainer, emergency paramedic or licensed physician selected by the program coach, to hospitalize, secure proper treatment, anesthesia or surgery for my child in an emergency. I also give the program coach permission to advise the hospital of our insurance information at the time of any treatment.

NFC Youth Summer League Basketball Program and its staff do not assume liability for any injuries incurred while playing.

Parents should contact their own insurance carrier to get additional insurance for the player if necessary.

As a condition of enrollment, the following disclaimer of liability must be signed and dated by the player's parent(s). The Player, in participating with NFC Youth Summer League Basketball Program and using the approved gym facilities, does so at his / her own risk. NFC Youth Summer League Basketball Program and its staff shall not be liable for any damages arising from personal injury sustained by the player during practices or games or at the facilities that we use. The player and his / her parent(s) assume full responsibility for any damages or injuries which may occur to the player during the season and so hereby fully and forever exonerate and discharge NFC Youth Summer League Basketball Program and its approved gym facilities, staff and coaches from any and all claim demands, damages, rights of action or causes of action, present or future, whether the same be known, anticipated, or unanticipated, resulting from or arising out of the player's participation with NFC Youth Summer League Basketball Program and in the use of the approved facilities.
